Request for Special Dietary Needs Accommodations
The U.S. Department of Agriculture's (USDA) nondiscrimination regulation (7 CFR 15b), as well as the regulations governing the National School Lunch Program and School Breakfast Program, make it clear that substitutions to the regular meal must be made for children who are unable to eat school meals because of their disabilities, when that need is certified by a licensed physician.
Hillsdale Community Schools follows the guidelines set forth by the USDA concerning the accomodation of special dietary needs of our students. Substitutions will be made to the best of our ability if a student has the necessary paperwork on file. If your child has special meal requirements due to allergies, a disability, or a disease, please follow the link below to access the form. This form should be completed by licensed physician and returned to the school your child is attending. Hillsdale Community Schools is not able to make any menu changes for your child until the form is on file.
